  • Print the form
  • Fill it out with the exception of the health related info.
  • Parent or Guardian signs it
  • Take a picture of it  
  • Send picture to trekwenatchee@gmail.com 


2)   Registration Form:  

        You will need the following information to fill out the form:
  • Personal info - name, birthday, phone numbers...
  • Medical concerns and medicines that you take
  • Food allergies 
  • Your doctors name and phone number
  • Health insurance provider and policy number
